Total Laparoscopic Hysterectomy(TLH)

TLH is the laparoscopic removal of the uterus and cervix. Removal of the ovaries and tubes depends on the patient. This surgery is done using a laparoscope, which is a slender, fiber-optic tube equipped with a miniature camera, lights and surgical instruments inserted through few small cutsless than one-quarter to one half an inch in length made in the belly. The surgeon performs the hysterectomy viewing the operation on a video screen. A traditional open hysterectomy requires an abdominal incision of four to eight inches.

Even a large uterus can be removed laparoscopically using this technique. Women who have had multiple pelvic and abdominal surgeries can also have TLH and we have very good success rates in such cases.

A woman may have a hysterectomy for different reasons, including:

  • Uterine fibroids that cause pain, bleeding, or other problems
  • Uterine prolapse, which is a sliding of the uterus from its normal position into the vaginal canal
  • Endometriosis
  • Abnormal vaginal bleeding
  • Chronic pelvic pain
  • Adenomyosis, or a thickening of the uterus
  • Cancer of the ovaries

Post surgiery, patients have a much quicker recovery, usually going home after 1 day in the hospital. Often, patients are able to return to their normal routine in one to two weeks. Patients report less pain, minimal post-surgical narcotic pain medication use, and a faster recovery time than women undergoing abdominal hysterectomies who usually require a three to four day hospitalization and lengthy recovery time of usually six to eight weeks.

Laparoscopic Supracervical Hysterectomy (also called Subtotal Hysterectomy): Removes the uterus but leaves the cervix in place, which some research suggests may reduce the risk of pelvic organ prolapse and preserve sexual function.

Laparoscopic Radical Hysterectomy: May remove all of these organs (the uterus, cervix, fallopian tubes, ovaries), plus the pelvic lymph nodes. This is performed in case of cancer of the uterus and cervix.
